Why Have Cosmetic Surgery After Weight Loss?

Picture a balloon that has been inflated and deflated. Once the material has been stretched, it doesn’t simply snap back into place. The same thing happens with our skin.

Massive weight loss is defined as the loss of more than 100 pounds or 100 percent above the person’s ideal body weight. Losing this much weight often leaves behind substantial amounts of loose, hanging skin because the tissue lacks the elasticity to shrink back to its former shape.

This excess skin isn’t just a cosmetic concern. Many people find that it causes physical discomfort, with issues ranging from skin irritation and rashes to difficulties with exercise and daily activities. The excess skin can also make it challenging to find clothes that fit well, practice good hygiene or maintain proper posture.

Beyond the physical aspects, the excess skin may affect your confidence in social and professional situations. You’ve worked hard to reach a healthier weight, but excess skin can prevent you from fully enjoying and celebrating your transformation. Some patients even report feeling trapped in their new body, dealing with physical reminders of their former weight despite their incredible achievement.

Post-weight loss cosmetic surgery addresses these concerns by removing excess skin and reshaping your body contours.

What Types of Surgery Do People Have After Weight Loss?

Most post-weight loss plastic surgeries focus on removing and tightening excess skin, as well as addressing exercise-resistant fat. The procedures included in your treatment plan will depend on your specific needs and goals.

Arm Lift

An arm lift (brachioplasty) addresses the loose, hanging skin that often develops on the upper arms after weight loss. Dr. Mariotti removes excess skin and fat from the area between your armpit and elbow, creating a more toned and defined arm contour. This procedure helps eliminate the “bat wing” appearance that can make you self-conscious about wearing sleeveless clothing.

Tummy Tuck

The abdomen often has the most skin laxity after dramatic weight loss. A tummy tuck (abdominoplasty) removes loose skin and excess fat while tightening weakened abdominal muscles that may have separated during weight gain. It can also remove stretch marks and C-section scars below the navel. The ultimate result is a flatter, firmer midsection that better reflects your weight loss success.

Lower Body Lift

A lower body lift, sometimes called a belt lipectomy, provides a complete transformation of your lower body contours. This procedure addresses sagging skin around your entire midsection, including the abdomen, hips, lower back, buttocks and thighs. Dr. Mariotti extends the tummy tuck incision so that it circles your body like a belt, allowing him to lift and tighten these areas comprehensively.

Breast Lift

Weight fluctuations can leave breasts deflated and sagging. A breast lift repositions your breast tissue higher on your chest while removing excess skin, creating a more youthful shape and position. Sometimes excess tissue from the lateral chest wall can be used to augment your breast volume. Dr. Mariotti carefully sculpts your breast tissue to restore natural-looking proportions that complement your new figure.

Breast Reduction

Some patients find that even after weight loss, their breasts remain disproportionately large. A breast reduction removes excess breast tissue, fat and skin to create smaller, lighter breasts that better match your new body proportions. This procedure can also alleviate physical discomfort and limitations caused by overly large breasts.

Thigh Lift

A thigh lift addresses loose skin on the upper inner and outer thighs that can cause chafing and make it difficult to find properly fitting clothes. Dr. Mariotti removes excess skin and fat to create smoother, more proportionate thigh contours that allow you to wear shorts and skirts with confidence. This is usually one of the last surgeries performed in a staged treatment plan.

Buttock Lift

Weight loss can leave the buttocks flat and saggy, with excess hanging skin. A buttock lift removes this loose skin and repositions the remaining tissue to create a more shapely, lifted posterior. This procedure helps restore natural curves while eliminating skin that can cause hygiene concerns and clothing difficulties.

Facelift & Neck Lift

While most post-weight loss concerns are body-focused, weight loss can also accelerate facial aging, leaving you with loose skin around your cheeks, jawline and neck. A facelift and/or neck lift repositions deep tissues and removes excess skin to restore more youthful and defined contours. Dr. Mariotti takes care to create natural-looking results that don’t appear pulled or tight.

Liposuction

Even after losing a lot of weight, you might find yourself dealing with stubborn pockets of fat that resist diet and exercise. Liposuction can remove these isolated fat deposits, helping to smooth and contour specific areas of your body. This procedure is often combined with lifting surgeries to enhance the overall results.

What Is the Difference Between Bariatric Surgery and Post-Weight Loss Surgery?

Bariatric surgery, such as gastric bypass or gastric sleeve procedures, helps initiate weight loss by restricting how much food you can eat or how your body processes it. These procedures are performed by bariatric surgeons. Here in the East Bay, we have one of the best bariatric surgery centers in the nation located at John Muir Medical Center in Concord.

Post-weight loss surgery is performed after you’ve achieved your weight loss goals. These cosmetic procedures, performed by a Board-certified plastic surgeon like Dr. Mariotti, address the after-effects of major weight loss, particularly excess skin that won’t respond to diet or exercise.

Put simply: bariatric surgery helps you lose the weight, and post-weight loss surgery helps reveal the results of that weight loss by contouring your body. They’re complementary procedures that work hand in hand but occur at different stages of your journey and require different surgical expertise.

Dr. Mariotti has performed major contouring procedures on many of the bariatric surgery patients not only from John Muir Medical Center, but also from Stanford, San Francisco and San Diego, and many from out of state.

Dr. Mariotti and his entire team are amazing. 10/10 on all fronts. Exceeded every expectation I had. My stomach is flat. Flat, flat. The stitching and incisions are so small people are flabbergasted when I show them. He is an artist to be sure.

Preparation

During a private consultation at our Concord plastic surgery office, Dr. Mariotti will evaluate your areas of concern, listen to your goals, and discuss your weight loss journey, lifestyle and overall health history. This conversation is the foundation of your customized surgical plan, which might include multiple procedures performed in stages for optimal safety and results.

You will receive detailed preoperative instructions to help ensure you are in the best physical and mental shape possible for your surgery. Our team will work with you to schedule your procedures and explain our financial policies and payment options.

Procedure

Your surgery will take place at Premier Surgery Center in Concord, where Dr. Mariotti serves as medical director. The length of the procedure will depend on which combination of surgeries you’re having. It is likely to range from two to six hours under general anesthesia. Some patients choose to have multiple procedures performed at once, while others prefer to stage their surgeries over time.

Dr. Mariotti takes a methodical approach to body contouring, carefully removing excess skin while preserving important supporting structures and blood supply. He places incisions so your scars will be as discreet as possible and hidden by most clothing. His experience in both reconstructive and cosmetic surgery allows him to achieve natural-looking results while prioritizing your safety and comfort.

Recovery

Recovery from surgery after weight loss varies significantly based on which procedures are performed. Most patients take at least two weeks off work, with more extensive procedures requiring longer recovery periods.

Expect some swelling, bruising, tenderness and discomfort as your body begins the healing process. Dr. Mariotti will prescribe the appropriate medication for pain management. You should notice an improvement in these symptoms, as well as your results taking shape, over the next several days and weeks.

Initially, you may need to wear compression garments to minimize swelling and support your healing tissues. While you’ll be encouraged to walk shortly after surgery, you’ll need to avoid strenuous activity for several weeks. You may be able to resume light physical activities around two to three weeks post-surgery and more vigorous activities at the four- to six-week mark.

Dr. Mariotti and our caring team will monitor your progress through regular follow-up appointments to ensure your recovery stays on track.

Results

Your new figure will emerge gradually as swelling subsides and tissues settle. While you’ll notice an immediate improvement in your body contours, your final results typically take six to 12 months to fully develop. During this time, your scars will continue to fade and soften, and your body will adjust to its new contours.

The results of post-weight loss body contouring are long-lasting if you maintain a stable weight. Surgery is never a substitute for a healthy lifestyle. Support your results through appropriate diet and exercise and healthy habits like adequate hydration and sleep, responsible alcohol consumption and stress management.

Is Post-Weight Loss Surgery Right for Me?

The aesthetic benefits of post-weight loss surgery are undeniable, and beyond the physical changes, many people report significant improvements in their quality of life. It’s a joy to see how our patients carry themselves differently afterward—they stand taller, move more freely and finally feel like their outer appearance matches the healthy, vibrant person they’ve become on the inside.

The best candidates for surgery after weight loss are in good overall health, don’t smoke or are willing to quit, and have realistic expectations about what surgery can achieve. If you’re still in the process of losing weight, Dr. Mariotti may recommend waiting until you’ve reached and maintained your goal weight to ensure the best results.

Your experience with post-weight loss surgery is deeply personal, and there’s no single “right” approach. Dr. Mariotti will help you create a custom treatment plan that balances your goals, lifestyle, schedule, budget and recovery preferences. Whatever your needs, we’re here to support you in completing the transformation you’ve worked so hard to achieve.

You might be a good candidate for post-weight loss surgery if you have achieved significant weight loss and:

FAQS

How much does post-weight loss plastic surgery cost?

The cost of plastic surgery after weight loss will depend on which and how many procedures you choose to include in your surgery. Combining multiple procedures into one surgery will help to offset fees for anesthesia, operating room facilities and other associated costs.

In some cases, insurance may cover all or part of the cost of your after-weight-loss plastic surgery. Check with your insurance provider for more information.

If cost is a constraint for you, discuss your financing and payment options with Dr. Mariotti at your consultation.

When is the best time to get plastic surgery after weight loss?

The ideal candidate for plastic surgery following weight loss will have already come close to or achieved their target weight prior to plastic surgery.

Before pursuing any post-weight loss procedures, Dr. Mariotti recommends that you maintain a stable weight for at least six months. This stability ensures that your results will be long-lasting and that your body has fully adjusted to its new size. For patients who have undergone bariatric surgery, we typically recommend waiting at least 12 months after the procedure so your weight can stabilize completely.

Can I view before and after photos of people who have had post-weight loss surgery?

We love celebrating our patients’ success stories. Our online photo gallery includes photos of previous clients, taken and shared with their consent, who have had surgery after weight loss in the Bay Area. You can also see photos on our Instagram and Facebook.

How do I choose an after weight loss plastic surgeon near me?

Look for a Board-certified plastic surgeon who has experience with post-weight loss treatment plans and procedures. Consider, too, the specific procedures you’re thinking about including, looking at before and after photos from this particular plastic surgeon.

Will I have visible scars after post-weight loss surgery?

Post-weight loss surgery does result in scars, but Dr. Mariotti takes great care to place incisions where they can be hidden beneath most clothing and swimwear. The length and location of the scars depend on the procedures performed and the amount of excess skin removed. Although scars are permanent, they typically fade over time and continue to improve for up to a year after surgery. Most patients find that the benefits of removing excess skin far outweigh their concerns about scarring.

Will I need drains after post-weight loss surgery?

Dr. Mariotti offers a no-drain technique for abdominoplasty and lower body lift. Previously, external drains were put in place following surgery in order to minimize fluid buildup, called seroma. Though these drains are useful, they can be inconvenient and uncomfortable, affecting mobility and causing additional stress during the recovery period.

With the no-drain technique, internal sutures are used to secure the skin to the abdominal wall, thereby closing the space where fluid could collect. The internal sutures are dissolvable and do not require removal.

During your consultation, be sure to ask Dr. Mariotti about your candidacy for a no-drain procedure.

I haven’t seen my body look like this since my 30’s.  And while I did lose over 10 lbs of fat and skin, it’s the non-scale victories which are the BEST. Like being able to bend all the way down in the gym on the machines (no apron roadblocking me), and doing the same while painting my toenails. I would highly recommend Dr Mariotti and his entire staff.”
