Terms and Conditions
- The referred patient must provide your full and complete name at the time of initial consultation or service.
- Referral Gift Cards can be applied to surgical and MSpa services.
- Referral Gift Cards are non-transferable, may not be redeemed for cash, and are not applicable to skincare or beauty products.
- The Patient Referral Program applies to patient referrals made after November 1, 2018.
- Your Referral Gift Card will be issued after the referred patients receive their services.
- Aesthetic surgery services referrals must be a minimum of $1,500 to qualify for the program.
- MSpa services received must be a minimum of $250 to qualify for the program.
- Referral Gift Card for each completed surgical service = $100.
- Referral Gift Card for each completed MSpa service = $25.
- Referral Gift Cards can only be applied to future surgeries or MSpa Services. They cannot be applied as a payment on an outstanding balance or previous surgery or service.
- Insurance surgeries or services are excluded from the Patient Referral Program.
- Physicians and physician offices are excluded from the Patient Referral Program due to regulations.
- Referral Gift Cards are only available to patients of Dr. Mariotti and/or the MSpa.
